On-site disposal log

Root balls, stems, fan leaves, seeds and stalks may be disposed of on site (burning, incineration, burial, mulching, composting). Log the license, material, method, date and time, and employees, with a signed attestation under penalty of perjury. Keep the log 5 years.
Why it matters
An unsigned log is an incomplete one.
